
The IMNDA has put together a health information passport, with help from the MND Association, England, Wales & N. Ireland. They allowed us to use their passport template and we tailored this document to suit the needs of the Irish MND community. We would like to acknowledge our own registered members of the MND community for their input also. This aim of the health information passport is to assist the health care professional (HCP) in understanding the needs of the person living with MND during their health care visits.
The passport allows the person living with MND to fill out their personal details and personal needs in one place and have this information to hand during their health care visits. It includes information such as: name, documents, communication needs, positioning, medication and so on. This document ensures that the health care professional is aware of their patients needs particularly where there is communication difficulty.
